Registration Form for MIP

 

The Northeast Hispanic Region is very excited to join you on this journey to acquire your CAMS certification. This is the next step in achieving your ministry goals within our denomination.

This form will serve as your registration for that program. Once the information has been delivered to the DOE1, you will receive an email from the Church of God giving you the instructions to sign up for the courses you will be a part of.

Please make sure your email address and postal address are correct when you use this form to sign up for the program you are applying for. (Failure to provide accurate information may delay your registration)

Married, (1) seeking credential: $765
Married, both seek credentials:$825

*To begin this program, we must receive FULL PAYMENT.
